Job description: The Geriatric Emergency Management Nurse (GEM) serves as an expert clinical resource in the Emergency Department (ED). The GEM practices within the Interprofessional team model and is responsible for promoting best practice in the care of older adults and excellence in the nursing care of seniors in the ED. Their focus is on frail seniors who are at risk of suffering adverse events, loss of independence and admission to hospital or long-term care. GEM nurses provide comprehensive nursing assessments and care for patients over the age of 65 in the ED. The GEM enhances linkages with hospital and community services and are advocates for the frail seniors they serve. In their role as geriatric educators and capacity builders within the ED, the GEM nurse helps to build the system’s capacity to ensure that frail seniors at risk receive the right care, in the right place and at the right time.Responsibilities Include:

• Completing comprehensive assessments of patients aged 65 years and older in the ED using appropriate screening tools
• Communicating assessment findings and recommending interventions to the interdisciplinary team including the most responsible provider
• Collaborating with the interdisciplinary team and community agencies to facilitate appropriate referrals and discharge recommendations
• Providing patient and family inclusion in intervention planning and execution
• Critically analyzing and incorporating research/evidence into clinical practice
• Providing formal and informal education to internal and external stakeholders
• Participating in program evaluation, reporting requirements, committees and quality assurance initiatives

Education/Certification:

• Current Certificate of Registration from the College of Nurses of Ontario in good standing (Registered Nurse)
• Gerontological Nursing Certification required (CNA) preferred
• Bachelor’s degree in nursing from a recognized University
• Master’s degree in nursing or a related health care field, completed, or near completion is considered an asset
• Current BCLS required
• Member of professional body preferred (Registered Nurses Association of Ontario, Canadian Nurses Association)
• Certification in Gerontology (such as CNA or post RN certificates with gerontology focus) required or willingness to undertake within year of start date
• Education in utilization management, case management, research methodology and data analysis preferred

Experience:Minimum 3-5 years relevant clinical experience in geriatrics or emergency servicesExperience in Emergency Services considered an assetCompetencies:

• Knowledge and proficiency in current evidence-based methods and practices with an emphasis on health promotion and risk reduction
• Demonstration commitment to continuous quality improvement
• Understanding of trauma, social determinants of health and addictions
• Flexibility to adapt to change and ability to work independently when necessary.
• Ability to minimize risks adhering to established policies and procedures.

Other:

• Computer literacy in Microsoft Word, Excel, and Outlook
• Experience working within MediTech Expanse Systems Electronic Medical Record an asset
• Satisfactory criminal reference and vulnerable sector screening prior to hiring
